Community Services


We provide a wide range of services for adults within the community across Cumbria.

These services are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and UHMBT acute sites.

Our community services work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Cardiac Rehabilitation Service - helps people recover and get back to their usual lifestyle after a heart attack or cardiac surgery. 

Case Management Teams - a team of various healthcare professionals who coordinate the healthcare of people across the Integrated Care Communities (ICC’s) with multi morbidity needs.

Community Respiratory Service - provides specialist respiratory care and assessment and pulmonary rehabilitation.

Continence Service - specialist nurses provide high quality treatment, care specialist products for people suffering from bladder and bowel problems.

District Nursing - services provided across South Cumbria as a 24 hr service for housebound patients who need wound care, crisis intervention and complex health needs, end of life care and case management.

Heart Failure Team The Heart Failure Team provides specialist care and advice to patients, their family members and their carers about Chronic Heart Failure. 

Integrated Rapid Response Service - provides a rapid response to deliver rehabilitation and recovery to people in their own homes, and to avoid hospital admission.

Millom Hospital – Millom community hospital has inpatient 'step up- step down' beds and provides a range of services.

Palliative CareWe provide part of the Cumbrian Specialist Palliative Care Service. The Specialist Palliative Care Team offers advice on pain and symptom control, psychological and spiritual support and advice to families and carers.

Community Physiotherapy - The domiciliary physiotherapy service is mainly for housebound patients or for patients who are best treated at home. The aim of the service is to support and  maintain people living in the community and preventing hospital admission. This may be through an episode of rehabilitation or supporting people to manage their long term health conditions. This service operates across the whole of South Cumbria, having teams in Millom, Barrow and Kendal. The service operated Mon-Fri  08.30-16.30. The service provides domiciliary physiotherapy, physiotherapy support to IRRS and physiotherapy input to the step step down Langdale Unit and the in-patient beds at Millom Hospital

MSK Physiotherapy provides a specialist services in the Furness area for people who have a musculo-skeletal problem. This services operate out of community facilities across Furness and Millom

Podiatry -  provides specialist foot care and works with other services including vascular, diabetes and biomechanics.

Speech and Language Therapy Service - helps adults who have problems communicating or trouble eating or swallowing.

Step up Step Down Units - where patients may be admitted to Community Hospitals from home (step up) for short term 24-hour nursing help, or from a hospital ward (step down) for extra nursing to facilitate a safer return home. Abbey View Unit in Barrow has 24 inpatient beds and the Langdale Wards at Westmorland General Hospital (North and South combined) have 46 commissioned beds

Tissue Viability Service - specialises in the prevention and management of complex wounds such as pressure ulcers or traumatic injury.
