The new digital Visual Impairment Hospital Passport is for patients who have a visual impairment.
Scan the QR code on screen to complete on your mobile device or click the link. You will need the patients NHS number to complete this passport.
For help finding your NHS number, go to: Find your NHS number (www.nhs.uk).
Upon completion, a copy of your new passport will be available to download and will also electronically send to your Hospital electronic health record.
If you cannot complete the digital version of the passport, please download a paper copy and then scan and send to: patientexperience@mbht.nhs.uk or post to:
