The cardiology team comprises of consultants with special interest and skills, advanced nurse practitioners, heart failure nurses, an arrhythmia nurse and cardiac rehabilitation nurses.

People suffering from heart disease can access the services in a variety of ways from emergency admission through to elective planned procedures.

In the community

The community heart failure/Arrhythmia service is a Nurse Practitioner led; clinic based service which aims to improve the care of patients diagnosed with heart failure through a model of shared care incorporating education, support and direct caseload management.

We also receive referrals for undiagnosed patients with possible underlying cardiac rhythm disturbances and investigate them accordingly offering an end treatment plan. We offer domiciliary visits also.

As a community based service which strives to maintain NICE quality standards, this requires a shared care approach.

The service maintains close links with both primary and secondary care staff. An educational advice and support system is embedded (Shared Care Model) through practice education and skill development.
