The Diabetes Specialist Care Team is a multidisciplinary team, including Consultants, Diabetes Specialist Nurses and Diabetes Specialist Dietitians. The team works in hospitals and community clinics to help adults with diabetes to manage their condition.
We can support you with:
- advice for the management of diabetes for hospital procedures
- carbohydrate counting
- diabetes and technology
- frailty/end stage renal failure
- insulin pump therapy
- steroid induced hyperglycemia
- pre-conception/ante natal care
Support for people in hospital
Our diabetes specialist care team works with University Hospitals of Morecambe Bay NHS Foundation Trust to identify adults with diabetes in hospital. The team provides expert care for these patients to support their early discharge and reduce re-admissions.
We also educate patients and other healthcare professionals on the management of diabetes in hospitals. The team are available Monday to Friday at University Hospitals of Morecambe Bay.

Support in the community
Through our network of community clinics, the diabetes specialist care team supports adults with type1 and type 2 diabetes. The team works with patients to promote a healthy lifestyle and reduce the complications associated with diabetes.
The team that works in the community are available Monday to Friday between 9am and 5pm.
