Diabetes and Endocrinology

The Diabetes and Endocrinology service supports both patients and primary care clinicians - such as GPs, practice nurses and district nurses - in the recognition and management of diabetes and diabetes-related complications, working as part of an integrated team in hospital and in the community. Services are provided in a variety of settings, and home visits are also made where patients are house-bound or are residing in residential care or nursing homes. Benefits of the service include:

• Prevention of diabetes-related complications
• Promoting and supporting patient self-management of their condition
• Reducing the amount of time people have to stay in hospital and preventing avoidable hospital admissions wherever possible.
