Clinical and IT professionals create bespoke healthcare questionnaires that can be completed by you via the PEP+, upon completion the results are sent straight to your electronic healthcare record so your healthcare team can review the answers when they see you. You can also download a copy of your answers for your own records.
Services will oversee what content gets added to your PEP+ Forms & Leaflets section so you should only ever see relevant content.
You’ll receive an SMS text message to notify you that a new form or leaflet is available to complete/view along with a link to login to the PEP+
