Stroke services

We are the Integrated Community Stroke Team across Morecambe Bay, providing therapy to Stroke Survivors who require rehabilitation

The service is made up of a range of staff including, Physiotherapists, Occupational Therapists, Speech and Language therapists, Assistant Practitioners and Rehabilitation Assistants. Please see ‘meet the team’ page.

The team cover both inpatient and community. Most Stroke survivors will be seen in hospital. The therapists will complete assessments to see what each person might need. If therapy is required, the team will talk to the Stroke survivor or their family about what this will look like.

Each person has different needs and goals and therefore each therapy / treatment plan is different. No patient journey is the same.

If you do have ongoing needs, you will be supported to create SMART goals:

  • Specific (Breaking down the goal in to step by step / smaller goals as needed to meet the end goal. These may also be known as short term and long term goals)
  • Measurable (How will you know you have met this goal)
  • Achievable (Is it achievable?)
  • Realistic (Is it realistic for the level of impairment?)
  • Timely (goals will have a set time frame to be achieved within and reviewed regularly)

Everyones goals will be reviewed regularly by those involved.

Referral Criteria

This service is for those who have stroke specific needs and goals. You can request referral via your GP or other health professional.

Once referred, your referral will be triaged. If appropriate, you will receive an initial assessment, but it may be that your needs are most suited elsewhere and you may therefore be signposted to another service.
