If you have been placed on one of our Patient Initiated Follow Up pathways, the patient engagement portal can help support you.

Hospital services are working with our in-house IT development team to provide improved content to help support you with your long-term condition. This includes things like:

  •  Health Care Questionnaires (these may be sent at set intervals to check your condition) 
  • Improved resource content; including websites, videos, and leaflets relevant to your condition or medication 
  • Contact options; Send a message to your healthcare team, book an appointment, or call or email 

Each specialty will offer a bespoke service dependant on the pathway you have been placed, with everything easily accessible via your PEP+ portal.
