Pain Management Patient Questionnaire

The Pain Management Patient questionnaire you are being asked to complete digitally is the pre-appointment questionnaire, which provides the clinician with key information regarding your pain that will then be discussed during your appointment. 

We advise that all patients read through this Patient Information Leaflet to give you additional information about why you are being asked to complete this questionnaire digitally and some of the benefits of completing it electronically.

Completing the questionnaire

  • It should take no more than 15 minutes for you to complete this questionnaire.
  • Once you submit your questionnaire, you will be unable to edit it.
  • There is an automatic save feature embedded within the questionnaire which will save your response to every question.

Good to know
You will be asked about any medication you are taking to manage your pain, so it would be best to have these readily available if you do not know them from memory.

Pain Management FAQs

Unfortunately, you are not able to edit your questionnaire once it has been submitted. We request that you review your answers before submitting to ensure all the information you have provided is correct.

For this pilot, it is mandatory for you to use Mi-PRES to complete your pain management questionnaire.

Pain Management stakeholders within UHMB who have the appropriate skills and qualifications will have access to your questionnaire. This includes the pain management consultant who you are due to see during the appointment. The completed questionnaire will then be stored on the Trust's secure Electronic Patient Record with the rest of your patient notes.

Yes, you are able to save your answers. If you wish to save your answers, please click the save button before exiting the questionnaire. You will then be able to resume the questionnaire where you left off.

Once you have submitted your questionnaire, you can be rest assured it has become part of your NHS clinical file and will be available for the clinician and other stakeholders who have access to your clinical record.
